What to expect on Your first Visit:
- Call the office to set up an appointment.
- You can request forms to be faxed or sent to you prior to your appointment.
- Reveal to the office staff the purpose of the visit. This will ensure the amount of time necessary for your appointment.
- Upon arrival, you will sign in under the appropriate doctor, the front desk personnel will instruct you.
- If paper work has not been completed, then new patient forms will need to be filled out in our waiting room and then given back to the front desk when completed and signed.
- At that time, a copy of your driver’s license and insurance card will be obtained.
- You will be escorted to a room for a consultation with the doctor.
- When the doctor completes the history/consultation, the next step is to evaluate you.
- An examination and possible X-rays of the area will follow.
- An explanation of the findings and appropriate treatment will be implemented.
- Allow at least 1 to 1 ½ hours for the first visit.
- A report of findings and a treatment plan will be recommended for your case.
